Pam Green is an evolved human from Chandra's list.

By entering his sister Shanti's name as a password, Mohinder Suresh is able to unlock a list of evolved humans. Pam's name is included on that list.

Pam appears on Chandra's list, though her status is not visible.
Pam also appears on NBC's interactive form of the map, which spells her name as "Pan Green" and notes she is deceased.
In a February 19th Q&A interview, Heroes writers Joe Pokaski and Aron Coleite confirmed that Pan Green and Pam Green are the same person.
