To answer your question, it depends on the situation. When you're on a talk page (user talk or just a main page's talk page), there's a + tab at the top to start a new section for discussion. Newer sections always go to the bottom. If you want to join in on a previous discussion, there's an "edit" link at the right-hand side of the top of a section.
