If there was a explosion right next to Meredith the fire would not burn her; the only injury she would get would be when she hit something hard. Fire has not harmed Meredith in the past; Ted Sprague did not harm himself; etc. - If the building fell in on her, then she could have been killed (not being killed from the flames directly). + The explosion would have blasted all concrete near Meredith away from her, making it less likely she died. - But there was plenty of the building above her to fall down on her. Even if all of the building that was above her went up away from her, which is not likely, it would have to fall back down again, with much of it landing on her. + Maybe the majority of the explosion was out and away from her, and not upwards. + This could be referring to the explosion that happened when Claire was a baby.
