Pam Green was an evolved human from Chandra's list.

By entering his sister Shanti's name as a password, Mohinder Suresh is able to unlock a list of evolved humans. Pam's name is included on that list.

Pam appears on Chandra's list. Though her location is not visible, her name is in white rather than red text, indicating she is alive.
Pam also appears on NBC's interactive map, which spells her name as "Pan Green" and notes she is deceased.
In a Behind the Eclipse interview, Heroes writers Joe Pokaski and Aron Coleite confirmed that Pan Green and Pam Green are the same person.
