Alex is capable of breathing underwater for an indefinite period of time. He used this ability to save a surfing accident victim from drowning, and he often uses this ability to win swimming events without pausing for breaths. (The Swimmer)

While breathing underwater, Alex extracts enough extra oxygen to sustain another person via "mouth to mouth" transfer for an unspecified period of time (Exposed).
